Phase 2 · Decision Investigation

The decision was approved.
Why is it back on your table?

Decision Investigation establishes what changed, when it changed and what authority supported it, so leadership knows where it stands before the next high-consequence decision.

The cost of decision uncertainty

The Organisation Keeps Paying for a Decision It Believes It Already Made

Repeatedly revisiting an approved decision can consume executive attention and delivery capacity without resolving the underlying uncertainty.

Executive attention is diverted

Senior leaders are repeatedly drawn back into a decision that should already be governing execution.

Capital remains exposed

Investment continues while uncertainty remains over what the approved decision actually is.

Delivery continues without clarity

Teams keep executing while the approved decision stays uncertain, contested or repeatedly reopened.
Before committing further, establish what happened to the approved decision.
How Decision Investigation runs

Twenty Working Days. Four Checkpoints.

Day 1

Commencement

Begins once scope, evidence access and interview access are confirmed.
Day 5

Approved decision established

What was approved, by whom, on what criteria and under what governance.
Day 10

Execution changes examined

What changed after approval, drawn from documents and executive interviews.
Day 20

Determination issued

Your board receives the Formal Decision Continuity Determination.
Between Day 10 and Day 20, decision continuity is tested across Decision Ownership, Decision Criteria and Reopening Governance.
What you receive

A Determination Your Board Can Defend

Not a workshop or a slide deck. A written Formal Decision Continuity Determination, addressed to your board.
Every finding is traced to the evidence it rests on, so each one can be defended.

Questions Boards Ask First

Do we need Phase 1 first?

Yes. Phase 2 follows a Phase 1 determination of Investigation warranted.

What do you need from us?

Scope, access to the records and access to the executives involved. Day 1 begins once all three are confirmed.

Who sees the findings?

Findings are provided confidentially to the commissioning organisation's authorised board representatives.

Is this an audit or an assurance opinion?

No. It is a determination on the evidence about one approved decision, not an opinion on whether the decision was right.

What does it cost?

The fee is confirmed following scoping and agreed in writing before commissioning.
